Bright Futures Encounter Forms for Health ProfessionalsPlease note that the Bright Futures Encounter Forms for Health Professionals, which were published in 1998, are out of print. Although they can be accessed here electronically, the forms are outdated and no longer consistent with current Bright Futures guidelines.

In their original format, the encounter forms provide a detailed outline of the content of care for 29 recommended health supervision visits (prenatal-21 years old). The information focuses not only on the physical aspects of health but also on the social, cognitive, and emotional development and the well-being of children and adolescents in the context of the family and community.
