 Please
note that the
Bright Futures Encounter Forms for Health Professionals,
which were published in 1998, are out
of print.
Although they can be accessed here electronically,
the encounter forms are outdated and no longer
consistent with current Bright Futures guidelines. Bright Futures Encounter Forms for
Health Professionals may
be used as models for creating customized forms.

Instructions
Choose
the form you would like to view from one of the pull-down
menus. The form you selected will be displayed in your
Web browser. You can simply print and use the version
displayed in your browser.
Or
you can click the "Download the . . . forms as PDF
files" links to download several forms at once.
To ensure that you will be able to download and read
the PDF files, please read "Using Adobe® Acrobat® Reader®" below.
Adobe® Reader®
Adobe® Reader® is
free software from Adobe® that lets you view and
print PDF files. If you do not have Adobe® Reader® on
your computer, please click on the "Get Adobe® Reader® " image
below to get the software. Once downloaded and installed
on your computer, click on the link to the PDF file you
are interested in viewing and/or downloading. If prompted,
indicate where on your hard drive you would like this
file saved. Once saved, open Adobe® Reader® and
from the toolbar, select File, Open, then choose the
saved PDF file. If you have problems downloading and/or
installing the Adobe® Reader® software, or viewing
pdf documents, please visit the Adobe® Reader® support
page.
