The HealthCheck
Provider Education System is based primarily on information from
the following sources:
 |
The HealthCheck
Manual and Periodicity Schedule developed by
the Department of Health Medical Assistance Administration
Office of Children and Families and the Office of Managed
Care. |
The
manual was developed in collaboration with many organizations
and agencies. The content of this manual is based on the
information available at time of publication and will be
reviewed and updated periodically.
|
 |
The Bright
Futures: Guidelines for Health Supervision for Infants, Children,
and Adolescents and its companion Pocket
Guide developed by Bright Futures at Georgetown
University. |
| |
The
guidelines are consistent with those of the American Academy
of Pediatrics and the American Academy of Pediatric Dentistry,
and were developed with the support of the Health Resources
and Services Administration's Maternal and Child Health
Bureau.
|
 |
Medicaid
and EPSDT information from the Centers
for Medicare and Medicaid Services. |
| |
This
resource contains information on the EPSDT program including
periodicity schedule, components of an EPSDT visit, state
reporting requirements, and national CMS Form-416 data. |
