and 12 Years
The
routine 11- and 12-year preventive health visits offer ideal
opportunities to:
- Administer
the following immunizations:
- Tetanus, Diphtheria,
Pertussis (Tdap)
- Human
Papillomavirus (HPV-3 doses)
- Meningococcal
(MCVA4)
Ensure
that the following immunizations (or series of immunizations)
are up to date:
- Administer
vaccines not previously given
- Review
the health history and administer other vaccines recommended
for adolescents at risk, including:
- Pneumococcal
(PPV)
- Influenza
(Yearly)
- Hepatitis
A (HepA) series
13
through 18 Years At all adolescent visits from 13 through 18 years, providers
should:
- Ensure that the following immunizations (or series of immunizations)
are up-to-date:
- Tetanus,
Diphtheria, and Pertussis vaccine (Tdap)
- For
females: Human papillomavirus (HPV) series
- Meningococcal
vaccine (MCV4)
- Hepatitis
B (HepB) series (if no documentation that series
of 3 vaccines has been
completed)
- Inactivated
poliovirus (IPV) series
- Measles,
Mumps, and Rubella (MMR) series
- Varicella
(chickenpox) series
- Administer
catch-up immunizations for vaccines not previously
given
- Review
the teen's health history and administer any other vaccines
recommended for adolescents at risk, including:
- Meningococcal
vaccine (MCV4)
- Pneumococcal
(PPV)
- Influenza
(Yearly)
- Hepatitis
A (HepA) series
