forms were originally designed to help parents
become active partners in their children's health by translating
the health supervision visit into everyday language. Families
learned what to expect at each well-child visit and how
to prepare for the next visit, the types of questions and
concerns to talk over with their provider, and
ways to prepare for their child's next stage of development.
The encounter forms, available
online in English and Spanish, are
offered here as models for creating encounter forms for
use with families.
